Care Consultant: Santa Barbara

Position Title: Care Consultant

General Summary:

Provide information, referral, and training to people with Alzheimer’s disease and related disorders, as well as their caregivers and family members. This position requires a passion for working with and improving the lives of people with dementia and their families.

Essential Job Responsibilities

1.      Care Consultant

• Providing individual and family care consultations / coaching sessions either in person, by telephone, or via e-mail.
• Evaluate individual client situations and make recommendations to appropriate Alzheimer’s Association services and / or other available providers.
• Maintain short and long-term follow-up contact with caregivers through written, verbal, electronic communication, and in-home site visits.

2. Office Administration

• Develop, coordinate, and provide continuing support to our Resource Groups.
• Maintain forms and records of all client activity.
• Answer phone and relay calls and messages.
• Maintain and distribute family services materials for all chapter offices.
• Manage library use.
Minimum Requirements

• Significant experience working with persons with dementia.
• A familiarity with community resources associated with elder and dementia care, to better assist our clients in developing their care network.
• Bachelor’s degree in social work, human services, or related concentration
