    There's help available when it comes to navigating dementia care. Caregivers and people living with Alzheimer's or another dementia can access personalized dementia care for free, including a dedicated care navigator and tailored resources.

    Comprehensive benefits are available

    Dementia care is a team effort. You'll have access to Alzheimer's Association resources and expertise, with Rippl's clinical care team and a dedicated care navigator providing support at every step. Services may include: 

    Comprehensive assessment and care planning.

    A dedicated care navigator to help navigate the complexities that come with a dementia diagnosis.

    Ongoing assistance, including caregiver resources and a 24/7 support line.

    Care coordination, transitional care management and referrals for services and support.

    Clinical support, including medication management and counseling.

    Respite care services provide caregivers with a temporary rest from caregiving.

    How the free navigation service works

    We'll help you start the dementia care navigation process and work as partners every step of the way. Take the first step and contact us. 

    1. Call 855.271.2662 or fill out the form to get connected with a Rippl engagement specialist. They can help assess eligibility based on your Medicare coverage.
    2. After your eligibility is confirmed, you will have an introductory virtual visit with your care team.
    3. Your dedicated care team creates a personalized care plan for the person living with dementia and their caregiver. The care plan may include care coordination, medication management and education resources.
    4. We work in partnership with your doctors to provide specialized dementia care and support.

    Insurance coverage for dementia care navigation

    The Dementia Care Navigation Service is free for people with Medicare Parts A and B coverage under the Centers for Medicare & Medicaid Services (CMS) Guiding an Improved Dementia Experience (GUIDE) model. This model provides comprehensive, coordinated dementia care to help improve the quality of life for people living with dementia and reduce caregiver stress.

    A Rippl engagement specialist can check your Medicare eligibility. Rippl accepts Medicare in the states we serve: Florida, Illinois, Missouri, Texas and Washington.
