Senior Day Services (SDS)  is a safe, supervised, activity-based program designed for adults with cognitive or functional impairments.

Based in newly renovated facilities, SDS offers “the next best place to home” by providing a caring, welcoming environment for adults with dementia, brain injuries or physical disabilities.

We're located adjacent to Chapter headquarters at 420 Chambers Street in Sioux City (between 4th and 6th streets, just east of downtown).  

Hours: Open weekdays, 7:00 am - 6:00 pm.

Our goal is to help clients remain independent at home for as long as is safely possible, keeping families together and delaying final placement in a residential facility by an average of one year.

Certified by the State of Iowa*, SDS has been serving the Siouxland area since 1995.  SDS demonstrates recognized best practices in adult day services and offers extensive experience in providing individualized, person-centered care to clients throughout the tri-state area.
