Order Your Alzheimer's Association referral pad

The Alzheimer’s Association Houston and Southeast Texas Chapter
recognizes the need for patients and/or family members and caregivers of patients with
memory loss, Alzheimer’s Disease, or other types of Dementia to receive emotional
and informational support. In order to allow for a rapid connection to our services
and resources, our chapter has designed an easy-to-use Physician Referral Form for your convenience.

This form allows you to make a direct HIPAA compliant referral to our
chapter by faxing it to the Alzheimer’s Association Houston and Southeast Texas Chapter
24/7 HELPline at 713-314-1316.


The Alzheimer's Association has recommended assessment tools to help
clinicians detect cognitive impairment during Medicare Annual Wellness Visits.
