Support Group Attendance Reporting

We are changing how we report attendance and are in the process of creating a new database that will be much easier to use. In the meantime, to report your attendance please download and complete the Support Group Attendance Sheet below. Fill it out and save it, and use it to mark attendance each month. Email the completed form each month to mtucker@alz.org
 
Our policy regarding membership information

We know that many people are concerned about what is being done with the information collected on the people who receive our services.
 
  • Please download and read the Alzheimer’s Association Policy on constituent information and why we collect it. This letter explains our policy and can be distributed to your members.

Listing Update Request
This form is for current support group facilitators who are requesting to change information about their group, e.g. the time the group meets and/or the facilitator’s contact information.
