The health care community plays an important role in coordinating the medical care of their patients and families who are affected by Alzheimer’s disease. The Alzheimer’s Association believes that building relationships with physicians and other health care staff enhances patient care. We are committed to assist those affected by dementia and to welcome the challenge of empowering your patients and their loved ones to navigate through the steps of this disease.
The Alzheimer’s Association, Michigan Great Lakes Chapter consists of 23 counties in Southern and Western Michigan, and provides a variety of services to those with dementia, their caregivers, family, friends, and healthcare professionals.
Chapter services include:

  • Toll-free Helpline – (800-272-3900) available 24 hours a day, 7 days a week by trained staff who are knowledgeable about the disease.
  • Support Groups – facilitated by trained professionals and offers confidential and supportive environments for mutual sharing
    Education, Workshops and Speakers Bureau – provides presentations on memory loss, Alzheimer’s disease, dementia care, caregiver issues, and services of the Association
  • Information and Referrals – provides information on all aspects of the disease process and available community resources
  • Care Consultations – offered via telephone, scheduled or walk-in appointments. Off site appointments can also be arranged.
  • Medical Alert + Safe Return – offers a national identification program to assist in the safe and timely return of memory impaired individuals with propensity to wander

We encourage you to consider using the fillable Program Services Referral Form.  The form details the supports and services we can provide your patients.  Your presentation of this referral form with your patient’s signature will result in our reaching out to that patient to provide information, support and resources.  Completed forms can be sent via email to or faxed to 734-369-2816 for Alzheimer’s Association follow up.
