Every day, the Alzheimer’s Association Northwest Ohio chapter offers individualized education and care planning for caregivers and family members of individuals with Alzheimer’s disease or another dementia.

Because Alzheimer’s disease is a progressive disease, it is important for families to get help to understand what support they need now and plan for the future. A Care Consultation is an in-depth, personalized service for individuals and families who are facing many decisions and challenges associated with Alzheimer’s disease and related disorders. The goal is for each family to develop a better understanding of the disease, make a plan to secure needed care, and develop strategies for the best possible symptom management and communication. Because of COVID-19, consultations are provided over the phone. Our staff is also available to answer questions by email. Call 419.537.1999 to make an appointment. All consultations are free.

Other services we provide are:
·       Telephone support groups
·       Virtual education programs
·       24/7 Helpline at 800.272.3900.

Staff is available to serve you.
