    If you or someone you know is living with dementia or has memory concerns, the Care Consultation program may be the right fit for you.

    If you or someone you know is living with dementia or has memory concerns, the Care Consultation program may be the right fit for you. A counselor will work with you to discuss your needs and come up with a plan to meet them.

    This short-term program is offered by phone statewide and can take place in our office or in your home depending on your location.

    West Michigan Dementia Support Program

    The Dementia Support Program provides assistance to individuals and families caring for someone with dementia in Allegan, Ionia, Kent, Lake, Mason, Mecosta, Montcalm, Newaygo or Osceola counties. If you are caring for someone with memory loss at home and you or your loved one is aged 60 or older, you may be eligible for the Dementia Support Program. When enrolled, one of our licensed social workers will come to your home to help address caregiving concerns and behaviors as well as create a customized care plan, provide dementia education and connect you with community resources. Learn more.
